By signing this document below, you confirm that you acknowledge and accept a release of claims against Sparta Fight Club MK and agree to the following :
Understand that engaging in both supervised and unsupervised physical activities can be risky and that your are choosing to participate at your own risk. Each session begins at designated time, and we are not resposnible for students before the session starts or after it ends. Children must be supervised by their parents or legal guardians at these times.
You confirm that you do not have physical limitation, disabilities, or health issues that could be made worse by participating in exercise (unless you provide written consent from your doctor regarding any risk associated with your condition).
You accept full responsibility for any injuries, losses, or damages that may occur to you as a results of your participation in exercise.
You agree not to hold Sparta Fight Club MK or its instructors liable, nor will you seek compensation for any injury, loss or damage.
You consent to allow Sparta Fight Club MK to take and share photographs and videos of you or your child on our website or social media platforms.
I have read and understand all of the above information. I recognize that by agreeing to all of these terms, I will be allowed to participate in both supervised and unsupervised exercise.
